Description
Advanced practice nurses balance evidence, complexity, and compassion when caring for older adults with multiple long-term conditions. Management of these conditions has become increasingly complicated, yet best practices remain underutilized in many outpatient settings. This session guides participants in translating current evidence for hypertension, diabetes, and hyperlipidemia into person-centered clinical decisions. Using the Age-Friendly Health Systems 4Ms framework as the clinical lens, the session demonstrates how guideline-based care can remain individualized.
