Dr. Shari Isenberg-Cohen has been a geriatric nurse practitioner for over 25 years, practicing in a variety of clinical settings from long-term care, primary care, and now home care. She obtained a Doctorate in Nursing Practice (DNP) degree in 2015 from Rutgers University. For the past 15 years, she has been in four value-based organizations in leadership roles. Her last job was the assistant clinical director for ConcertoCare of North Carolina managing 10 APNs from Raleigh to Winston-Salem and clinical director of clinical care. She currently works for Optum as a house calls nurse practitioner. Her patients are the complex chronically ill elderly with serious mental illness and with social determinants of health.
