This in-depth focus session will focus on areas related to palliative care. Part 1: With the strong financial incentive of the CMS Age-Friendly Hospital Measure and the growing competition among Medicare-serving practices, delivering on the 4Ms (what matters, medication, mentation, and mobility) of age-friendly care has never been more important. What may be less obvious is the strong relationship between palliative care and each of the 4Ms. This section will explore using palliative care to help meet age-friendly requirements along with essential palliative care skills and a learning platform that has been widely used by APRNs. Part 2: This section will present an innovative outpatient palliative care program designed to serve patients in skilled nursing facilities, utilizing the Palliative Care Consultation Services model. It will describe the initial planning and development, factors for growth and maintenance of the program, common challenges observed in the long-term care setting, and the program’s influence on key metrics such as symptom burden, quality of life, and resource utilization across the healthcare system.
