Lani Hubbard is a hospice transition specialist and nurse practitioner for Bristol Hospice Hawai’i. She has worked as a geriatric nurse practitioner for over 20 years, with experience in hospice, palliative care, home-based primary care, sub-acute care, and long-term care. Lani earned a bachelor's degree in psychology from Dartmouth College and a master's degree specializing in gerontological nursing. Lani is a certified geriatric nurse practitioner and is a certified advanced clinical hospice and palliative care practitioner. Lani was the recipient of the Healthcare Association of Hawai’i Professional with Heart Long-Term Care award in 2012. Lani is currently serving as the GAPNA 2027 Pharmacology Conference Planning Committee.
