Ophelia Empleo-Frazier, a lecturer at Yale University School of Nursing (YSN) in the adult and geriatric primary care nurse practitioner specialty (AGPCNP), focuses on delirium, dementia, falls, deprescribing, and end-of-life care. Collaborating with the specialty program director, she restructured the curriculum to an “age-friendly” framework based on geriatric models of care and has incorporated and presented on Yale’s incorporation of PPC principles in their NP curriculum. As the owner of Frazier Consulting, LLC, she designed nursing course modules aligned with geriatric principles for the GWEP HRSA-funded Connecticut Older Adult Collaborative for Health [COACH4M] at Yale School of Medicine Geriatric Department. With over 30 years of geriatric care experience in diverse clinical settings, Ms. Empleo-Frazier is a board-certified geriatric nurse practitioner through the American Nurses Credentialing Center. She holds degrees from the Yale School of Nursing geriatric practitioner program and Saint Louis University in Baguio City, Philippines, and is a member of Sigma Theta Tau International Honor Society of Nursing, earning recognition for her nursing and philanthropic endeavors.
