Despite the critical importance of early Alzheimer's disease detection, only 8% of patients receive timely diagnosis. Healthcare systems require boundary-spanning roles to coordinate multi-stakeholder care and ensure efficient patient triage. This session will outline the implementation and outcomes of the brain health navigator (BHN) program, with specific focus on examples in which APRNs served as boundary-spanning coordinators to improve Alzheimer's diagnostic pathways.
