Advanced practice nurses are uniquely prepared to help patients and families navigate transitions of care. This session will highlight critical moments of intervention that can lead to better care and patient satisfaction. Whether an APRN works in a hospital, skilled nursing facility, or ambulatory care setting, it is important to understand the processes and limitations of each type of setting. We will examine clinical cases that depict both successful and unsuccessful care transitions and equip APRNs with practical tools to improve the patient experience.
