This session will describe the spoke-and-wheel model of an academic-practice partnership to address health inequity in underserved and rural populations, using a nurse-led mobile health team approach. Discussion will include the outcomes of the model regarding nursing student preparation for the role of primary care nurse in assessing and addressing social determinants of health and ways for attendees to replicate the spoke-and-wheel model in their own organization to prepare primary care nurses to assess and address health inequity in underserved and rural populations.
