Patient safety has traditionally focused on inpatient care, with less attention given to ambulatory care settings due to perceived control in inpatient environments. To establish a successful ambulatory care safety program, leaders must prioritize patient safety and dedicate a team to report monitoring and analysis. An interdisciplinary team, including nursing, advanced practice clinicians, physicians, practice leadership, risk management, coding and compliance, and patient relations, implemented evidence-based practice (EBP) interventions at one healthcare system based on the change management theory with the goal to reduce preventable harm by increasing event reporting by 5%, with a reach goal of 10%. This session will discuss the implementation and results of those interventions.
