Thanh Nguyen is currently the director of GI/GI surgery service line at Stanford Health Care since May 2023. Prior to this role, she was a nursing leader in primary care since 2016. As assistant director in the primary care service line, she led the transformation of Stanford Coordinated Care clinic to a care management model which is now called Stanford Coordinated Care Program (SCCP). This transformation allowed the team to touch more moderate/high-risk complex/chronic condition patients, support patients post admissions and transition home. Working directly with clinic operations and frontline teams to design, development, and implement efforts improving value-based care (VBC) and population health quality metrics.
