This session focuses on the challenges of anticoagulation treatment in older adults aged 85 and older. Discussion will include application of risk–benefit principles to determine appropriateness of anticoagulation; evaluation of bleeding and thromboembolic risk using validated tools and clinical judgment in patients; and development of individualized anticoagulation plans that incorporate comorbidities, frailty, fall risk, and patient goals of care.
