Achieving health equity is essential to ensure that every individual can reach their full health potential, yet numerous obstacles persist, particularly in the care of older adults. This session addresses the persistent disparities impacting healthcare access and outcomes among diverse communities. While some factors, such as genetics, are immutable, others, including healthcare quality, social and behavioral influences, and system-based inequities, can be transformed. This presentation empowers primary care providers to understand and apply health equity principles within their daily practice to reduce disparities and improve patient outcomes.
