Dr. Leslie Chang Evertson is the associate director for advanced practice providers at UCLA Health. She has been a dementia care specialist nurse practitioner for the UCLA Alzheimer’s and Dementia Care (ADC) program since 2012. She is a content expert and trainer for the ADC program model of dementia care dissemination effort funded by The John A. Hartford Foundation. She is co-chair of the healthcare system dementia care intervention and software content expert for a pragmatic trial of two models of dementia care (health systems-based and community-based). Leslie is a member of the UCLA Health Pride Network core foundational group, the UCLA Nursing Unity in Diversity Council, and a guest lecturer at the UCLA School of Nursing.
