Tifphany Hofstetter is a nurse care coordinator with the US Department of Veterans Affairs in Vancouver, Washington, where she has served for three years. She has worked in health care since 2007, beginning her career as a paramedic before moving into nursing, and holds a master's degree in nursing and healthcare leadership from Vanderbilt University. Her current focus is systemic process improvement and outcomes, with an emphasis on support that helps patients stay independent at home, prevent acute care admissions, and delay nursing home care. She and Noralynn Mayer have previously presented a poster and call to action on post-acute care at ANIA.
